Summary
Responsible for providing direct services of evaluation and treatment provided through Mercy Health Physicians. This position works with the providers and staff to collaborate in providing needed counseling, case management and social services for current patients/clients at various specialty physician practices in Youngstown. Support Palliative Care, community based (in home and facilities) and the outpatient clinic patients.
Essential Functions
40% Assesses and evaluates the needs of the client referred. Develops an intervention plan in conjunction with the client, family, physician, and other staff, and community agencies as appropriate. Provides brief intervention encounters, approximately 10-13 per day, utilizing both in-person and telehealth methods for interaction.
20% Maintains required accurate client records and documents progress notes, goals, assessments and other applicable data in a timely manner within the clients' charts.
20% Consults with providers and external resources and makes any necessary referrals for the ongoing care of the client.
10% Maintains knowledge of, and practices in accordance with professional practice principles, and with the legal and ethical guidelines of Mercy Health and the Ethical and Religious Directives for Catholic Health Partners. Maintains current knowledge about community resources including availability, eligibility requirements, and referral procedures in order to facilitate provision of services.
10% Coordinates with various employed physician practices daily as needs require. Participates in warm hand off protocols for clients, with internal and external stakeholders.
Education
Required: Master’s in social work, Counseling, Psychology
Licensure: Current license to provide independent psychotherapy services. Current OH licensure as a Licensed Independent Social Worker (LISW) required.
