About the opportunity
Mercy Health Albury Palliative Care Centre provides a community palliative care program that supports people in their home who have been diagnosed with a life limiting illness. Our aim is for each person to experience the best quality of life possible - physically, personally, socially and spiritually.
The role of the community registered nurse is to provide quality client-focused care to the patient, their family and carers utilising a holistic approach. The aim of the role is to maximise the quality of life of the patient and the family by appropriate provision of the highest quality symptom management and support, delivered in professional and timely manner. The community registered nurse will act as a role model and practice within the core values of Mercy Health.
This position will work collaboratively with the clinical nurse specialists, general practitioners, community health services, other health providers and community agencies to provide comprehensive, coordinated and collaborative care for clients/patients to improve their health outcomes.

To thrive in this role, you will have:
Excellent clinical, time management, organisational and communication skills, with the ability to adapt and problem solve.
Demonstrated skills and knowledge in case management, including care coordination for people living with life limited illness, using advanced clinical skills in patient/client assessment and care planning.
A compassionate, flexible and positive approach to effective patient care and management.
The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.
You will also be required to provide evidence of, or in the process of obtaining:
A current Police Record Check and Working with Children Check.
Current registration with the Australian Health Practitioner Regulation Agency as a Registered Nurse.
Evidence of current NSW driver’s license or equivalent from other Australian State, and ability and willingness to travel for work purposes.
Proof of current influenza vaccination and other vaccination status consistent with public health orders.
