The ideal candidate for this role is someone with experience and passion for stroke / neuro care; excellent attention to detail and organizational skills; someone who likes data and is able to not only extract data, but analyze and help develop action plans. This person will also help respond to stroke alerts and will provide education to patients and the community.

Schedule: Monday-Friday 7am-3:30pm

The Nurse Navigator manages the daily operations and workflow of the stroke program / neuroscience service line, in conjunction with the NS Program Manager, physicians, nursing staff, clinical support personnel, and administrative leadership. Position is responsible for acting as a patient liaison for the program, overseeing all patient-related clinical activities that parallel procedural operations. Works with physicians, referring physician offices, advanced practice providers, and other clinical personnel to manage all designated pre-procedure and post-procedure activities in their respective sub-specialty or discipline. Manage or oversees all aspects of their respective specialty clinic, including database data entry, statistical reporting, and quality tracking activities. Performs program quality assessment and reports findings to the physicians and administrative team.

  • Oversees all efforts related to guiding patients and families through the patient care continuum in their relevant clinic area; acts as a patient liaison and, working with the clinicians, performs participation selection, program invitation, scheduling, and outreach functions on behalf of the program. Under the guidance of the clinicians, performs clinical patient evaluation and provides input to the patient care process.
  • Oversees the scheduling of appointments and diagnostic studies; enrolls patients in clinical databases and studies, as deemed appropriate. Manages all aspects of the database(s), including data entry, data review and dissemination, and data/statistical reporting functions.
  • Communicates with referring physicians and coordinates information with referral services; acts as a liaison between other physicians and hospital departments.
  • Performs program quality assessment checks, audits, and reviews; develops and drives initiatives to eliminate barriers to care. Establishes processes and standards that ensure consistent and optimal patient navigation.
  • Plans and develops patient-centered programs; reinforces patient education and directs patients and families to available resources and support services.
  • Recommends and implements program guidelines; develops the required tools and materials to coordinate the patient care sequence.
  • Attends relevant conferences and meetings. Keeps abreast of current literature and keeps current on industry practice standards.
  • Works closely with communications and marketing/public relations to promote program outreach.

Required Minimum Education: 2 year/ Associates Degree

Preferred Education: 4 year/Bachelors Degree

LICENSURE/CERTIFICATIONS (must be non-expired/active unless otherwise stated):

Required: RN (current Ohio licensure), CPR, BLS, ACLS, NIHSS certification, 8+ (min) stroke education/year.

Preferred: BSN

MINIMUM QUALIFICATIONS

Minimum Years and Type of Experience: 3-5 years of clinical experience in a direct patient care environment, computer experience (Microsoft Applications)

Other Knowledge,

Skills and Abilities Required: In depth knowledge of specialty area. Demonstrates self-directed excellent communicaiton and customer service skills. Able to analyze/interpret data and relate results to the clinical performance of the specialty area.
