The Exercise Physiologist primary function is to provide overall consistent workflow within the Cardiac Rehabilitation Program. The Home Heart Health EP's primary function is to bridge the care from Inpatient to Outpatient Cardiac Rehabilitation. The goal is to access need for home intervention based on set parameters of evaluation, phone follow up, and home visit criteria. The EP will work in conjunction with the Home Health Nurse and Physical Therapist to promote optimal care and help eliminate readmissions. The EP will provide home visits for high risk participants to optimize exercise conditioning in the home setting prior to their outpatient Cardiac Rehabilitation.

ESSENTIAL FUNCTIONS

% Time

40% In home visits based on high risk readmission criteria and inpatient interviews. Provide assistance with ambulation and daily exercise routines to increase strength, endurance, education and reduction of anxiety. Work in conjunction with Home Health Nurse to merge care from Phase I inpatient rehab to Phase II outpatient rehab.

20% Inpatient evaluations with patient and family members to determine in home needs while developing an individualized exercise care plan from the exercise prescription along with providing the patient and their family with a curriculum of cardiac care increasing education and quality of care.

15% Follow up phone calls to all cardiac CABG and valve patients from mild to high risk based on set parameters.

15% Measuring outcomes from the patients that are seen in the home vs outcomes from patients receiving phone follow ups only. The following measures will be tabulated: readmission rates (short term with in 30 days),patient satisfaction, mortality rates, and anxiety levels.

10% Carepath charting on patients status, ITP, visits, phone calls and educational documentation.

MINIMUM EDUCATION

Required Minimum Education:

4 year/bachelors - Specialty/Major: Bachelor or Master Degree in Exercise Science

Preferred Education:

Masters/Graduate Degree- Specialty/Major: Bachelor or Master Degree in Exercise Science

Licenses Required:

Required: BCLS and ACLS

Preferred: ACSM certification as Exercise Specialist

Minimum Qualifications:

Minimum Years and Type of Experience: Previous experience and or internship

Other Knowledge, Skills and Abilities Required: Basic and advanced knowledge of cardiac arrhythmias, blood pressure monitoring, cardiac resuscitation, and exercise equipment knowledge

Other Knowledge, Skills and Abilities Preferred: diversity, accountability, and decision making skills
